Even the healthiest people need a yearly check-in with their doctor. Your Blue Shield of California plan covers an Annual Wellness Visit at no extra cost to you, and, even better, you can get rewarded for the visit.

With the Blue Shield of California Appreciation Program, you can earn a $50 gift card for having your Annual Wellness Visit. Just register for the program and complete your visit before December 31. To learn more, call (866) 352-0892 (TTY: 711), 8 a.m. to 6 p.m., Monday through Friday, excluding holidays.

What is the Annual Wellness Visit?

While you have Medicare coverage, you’re eligible for an Annual Wellness Visit. It is a dedicated time to meet with your provider, have a conversation about your health, assess your needs, and develop a specific preventive care plan for you. This comprehensive visit is more than just an annual physical. 

The Annual Wellness Visit includes:

• Health Risk Assessment: A questionnaire covering topics like your medications, personal and family medical history, height, and weight. You can complete it before your visit. Ask your provider’s office if they can send it to you by computer or by mail beforehand. It takes about 15 minutes to finish.

• Provider visit: You’ll meet with your provider to review the Health Risk Assessment and discuss your overall health. Your doctor will cover topics such as memory, falls, urinary issues, and more. This visit can take place virtually over a smartphone or computer. If you are interested in a virtual visit, ask your provider’s office about this option. 

• Prevention plan: You’ll leave your visit with a personalized preventive care plan listing tests and exams you’ll need to stay healthy. Work with your provider’s office to schedule follow-up appointments for care.
