Requesting a provider directory

Need a printed copy of your provider directory?

1. Use our find a doctor tool to look up providers and facilities near you.
2. Once you’ve found the provider type you’re looking for, save or email your search results as a PDF.
3. If you need additional assistance, please call the phone number on your member ID card to request a directory.

If you need a printed Medicare provider directory, please contact Medicare Member Services at (800) 776-4466 or email MemberSvcs@blueshieldca.com. With a Medicare Advantage HMO plan, you have a primary care physician (PCP) who will be responsible for providing you with basic and routine medical care. You will generally need a referral from your PCP to obtain care from specialists associated with the same physician group as that of your PCP.
