Health and Wellness Programs and Education for Medi-Cal Members
Blue Shield of California Promise Health Plan (Blue Shield Promise) wants to help keep you and your family healthy. We believe the more you know, the better you can manage your health.
We have programs and services to help you stay active and fit, manage chronic conditions like diabetes, keep a healthy weight, and eat healthy. We can also help you find classes, counseling, and support groups.
Health and Wellness programs for our members
Health and wellness portal
Reaching and keeping good health is a lifelong effort. We offer several online tools that can help you live healthier and manage chronic conditions.
Go to the Health and wellness portal to create your account. Once you create an account, you may complete the health assessment and have access to the online health tools.
Wellvolution
Wellvolution is Blue Shield’s digital care program which is free to all Blue Shield Promise members 18yo+. Get tools, apps, and support designed for you to achieve your health goals.
Diabetes Prevention Program
We’ve teamed up with Solera Health to offer you a diabetes prevention program. There is no cost to you. This program helps you learn how to make small changes to your daily routine to be more active and eat healthier.
If you qualify for this program, you will meet with a health coach and a small group for support, online or at a site near you.

Care Management Programs
Care Management has three programs available for our members. Blue Shield Promise finds members who meet certain program criteria and asks them to join. The programs are free and optional to use.
- The Complex Care Management Program helps members with complex conditions.
- The Disease Management Program helps members with conditions such as asthma, chronic lung disease, congestive heart failure, heart disease, diabetes, and depression.
- The Transitions of Care Program helps members after being discharged from the hospital.
These programs offer you education to better manage your condition(s). Our nurse works with you and your doctor to help you stay healthy and to avoid the hospital. Your nurse gives you support and education. They also help coordinate your care and make sure you get all your preventive health screenings. Initial Health Assessment (first health checkup)
Make an appointment for your first health checkup as soon as you become a Blue Shield Promise Medi-Cal member. During your first health checkup, you will receive a complete physical. This checkup will help your doctor get to know you better so she or he can provide you with complete care.
Pregnancy Program
Our care team offers personalized help to get you ready for a baby. They will help you prepare for a healthy pregnancy. We also offer support if you're dealing with a pregnancy loss. It’s free for eligible members.
Behavioral Health Services
Blue Shield Promise Medi-Cal provides mental health and substance use disorder services through primary care physicians (PCPs), licensed behavioral health providers, and county providers and agencies. We also provide behavioral health treatment services for children under the age of 21.

Our health education brochures provide you with information on how to stay healthy and manage your conditions. Brochures are available on the following topics: alcohol and substance use disorders, asthma, children’s health, diabetes, family planning, pregnancy, breastfeeding and parenting, high blood pressure, nutrition and weight management, immunizations, smoking cessation, and many more.

Living Well Information Line
Health information is a phone call away. Call our Living Well Information Line to get answers to your questions about cholesterol, weight management, exercise, healthy eating, and diabetes. Call Member Services and ask for the Health Education Department.
